FCHP Sleep Services Authorization Information

All requests for sleep services for Fallon Community Health Plan members require prior authorization from either Sleep Management Solutions/CareCore National (SMS/CCN) or Fallon Community Health Plan (FCHP). The following list identifies where the request should be submitted:

SMS/CCN provides authorizations for only the following FCHP lines of business:

Authorization Required from SMS/CCN
  • FCHP Direct Care/Select Care
  • Commonwealth Care
  • Major Medical
  • Fallon Senior Plan HMO


    For the following lines of business sleep-related service authorizations must be obtained directly from FCHP preauthorization department and not from SMS
